Pass on your contact details and our participant liaison officer will reach out to you within 24 hours to discuss your needs.
We will discuss what you’re looking for and your unique circumstances. When we have the details required, we will reach out to our team of occupational therapists and ensure we match you with the practitioner who can best fit you. We don’t want to waste your time, so if we feel our practitioners can’t meet your requirements, we will let you know early on.

Step 2: Customised Service Agreement

Once we have discussed your needs with the practitioner and we have a good match, it’s time to complete our referral form.
We need all of your NDIS details, such as NDIS number and plan dates. To complete this form, [click here]. We can assist you with this, or you’re welcome to proceed at your own pace.
With the information provided in the referral form, we will create a customised service agreement to ensure complete transparency. The service agreement provides all of our prices and policies so you can make an informed choice as to moving forward with us.

Step 3 - Onboarding
With the service agreement now complete, we will begin the onboarding process. We’ll look to schedule your first appointment, starting with a pencil booking.
As a registered NDIS provider, we follow a thorough onboarding process that includes developing a personalised support plan. This plan allows us to deliver ongoing therapy to the highest standard and track progress against your goals—ensuring you receive the greatest possible value from our services.
